2. Medical Release form  voorhees jr vikes football cheer physical form 2025.pdf completed, signed and returned by July 15th! 

3.  Copy of Birth Certificate with raised seal returned by June 30th if you did not participate in 2025.

6.  Warren County Midget Football League Parent Code of conduct.  Follow this link to complete sign off.  2025 Parent Code of Conduct
